To schedule an appointment, please click on the icon "Contact US" located on the left-hand side of this page to learn how to reach us. We prefer that all appointments be scheduled by phone. If a doctor's office will be scheduling the appointment, please have the patient also contact us prior to the visit so that there is no misunderstanding as to what the patient will need to bring to the visit.

IMPORTANT

When scheduling an appointment, we must have the following information:

The patient's name
The patient's birthdate
The patient's social security number
The parents' names and contact numbers
The nature of the child's problem
The referring primary care doctor's full name and fax number
The patient's insurance inforamtion

Cancel an Appointment

If you are unable to keep your child's clinic appointment, please inform us so that we may give another child the opportunity to be seen in your child's time slot. If you do not show up for a scheduled clinic appointment and call later asking to be rescheduled, you will be asked to wait until the next available appointment.
